چکیده
Abstract Introduction Outcomes of men with delayed presentation ischemic priapism (>24 hours) are poor. Management options include forming a proximal corporal shunt (poor evidence efficacy) or immediate penile prosthesis placement the associated increased risk erosion and infection. Penoscrotal decompression (PSD) is thought to relieve acute compartment syndrome prolonged (PIP) restores perfusion. Experience currently limited 31 reported cases from several centers in North America. Objective To further assess role efficacy PSD for PIP. Methods Retrospective review presenting priapism. Only following were included. Ischemic was confirmed by cavernosal blood gas biopsy at time decompression. Duration ischemia, prior management clinical outcomes analyzed. Change International Index Erectile Function (IIEF) score compared. Results Thirteen patients median age 45 years (IQR 37-43) presented between July 2019 June 2022. Median duration PIP 48 hours 38-56). Etiology idiopathic 5, iatrogenic recreational drug-related 6 sickle-cell disease (SCD) 2 men. All failed aspiration, irrigation intracavernosal phenylephrine distal corporo-glanular shunting (T-shunt Winter shunt). had successful detumescence 9 (69%) requiring bilateral PSD. Immediate pain relief achieved all but one man who very (120 where no fresh bleeding found despite He chronic SCD lost follow-up discharge. Another patient recurred within 24 unilateral successfully managed phenylephrine. without developed recurrence next day he proceeded insertion malleable on 2. Ischemic priapism Low flow (ischemic) priapism is a penile erection persisting beyond four hours unrelated to sexual interest or stimulation. This type of priapism causes a compartment syndrome of the corporal bodies with progressive hypoxia, hypercarbia and acidosis. Clinically, it is typified by progressive penile pain with rigid, tender corporal bodies and soft glans penis and corpus spongio...

To investigate the efficacy and safety of the corpus cavernosum-corpus spongiosum shunt (Al-Ghorab Shunt) plus intracavernous tunneling (CC-CSS+ICT) for prolonged ischemic priapism (PIP). Twelve patients with PIP were enrolled in this study. The mean age of patients was 38.3 ± 9.2 years old and the mean duration of PIP was 2.8 ± 1.0 days (range, 1.5-4 days). All patients received CC-CSS+ICT for...
متن کاملIntracorporeal Dilatation Plus Al-Ghorab Corporoglandular Shunt for Salvage Management of Prolonged Ischemic Priapism
Ischemic priapism (IP) is a urological emergency that requires early intervention in order to prevent irreversible structural and functional changes. The purpose of penile shunt surgery is to passage out ischemic blood in the corpus cavernosum (CC), restoring the normal circulation within these structures. Here, we present our first experience using modification of Al-Ghorab shunt procedure to ...
متن کاملAvoiding complications: surgery for ischemic priapism
Ischemic, or low-flow, priapism is among the most common and challenging urologic emergencies. Management of recurrent or refractory ischemic priapism is even more challenging, with increasing levels of risk for both the patient and the urologist. The goal of this commentary is to condense a career of experience (TF Lue) in the management of ischemic priapism into a concise, practical clinical ...
